Eligibility

Current guidelines generally require the applicant organization to be tax exempt under the Internal Revenue Service; organizations that are not tax exempt are listed as ineligible. Dr. Miltie/QC Healthcare is therefore not the natural direct applicant. The strongest pathway is an eligible Ohio nonprofit applicant that independently owns the project and selects appropriate vendors or partners.
Allowable Uses, Restrictions, and Match
The Foundation focuses on physical and mental health and has recent first-party grant history supporting direct healthcare services and equipment. Its official 2025 awards included a bladder scanner, audiometer, CPR device and power equipment, nursing equipment, bariatric training equipment, and exercise-science equipment. This history supports a credible applicant-led medical technology pathway where the project is otherwise eligible, but it does not guarantee that any particular device or expense will be approved.
The current guidelines do not state a mandatory match or cost-share requirement. They exclude non-tax-exempt organizations, annual appeals and membership drives, unspecified capital campaigns and fundraising events, program advertising, lobbying, and endowment. Incomplete or nonconforming requests are not eligible for consideration.
Submission Rules and Required Materials
Applicants must call the Foundation office at 330-580-2380 to request the current application and guidelines. The first required step is the Foundation’s Grant Request Cover Sheet, which is obtained from the office. The published initial package also requires an organization description, mission statement, project or program objectives, current-year budget, IRS 501(c)(3) determination letter, prior-year actual financial statement, and board list. The Foundation provides the detailed submission instructions after the applicant requests the materials. Incomplete proposals are not considered.
