The form requests organization and contact information, date and place of incorporation, tax exempt classification and number, organization purpose, number of people directly served, request category, project purpose, amount requested, total project cost, percentage of the grant to be used in Central Kentucky, five major funding sources and amounts, plans for completing the project if sufficient funds are not raised, political activity disclosures, records inspection consent, and prior Keeneland Foundation support. It requires upload of a board of directors list and the tax exempt determination letter. Each upload accepts one PDF, DOC, or DOCX file up to 50 MB.
