Eligible applicants

Eligible applicants are nonprofit and publicly funded EMS agencies operating in Kentucky. Applicants must hold an active EMS agency license and be in good standing at application and throughout the award period. Programs must serve rural Kentucky communities and comply with Kentucky Board of Emergency Medical Services standards.

Program focus and allowable uses

Community paramedicine activities may include in home and community based follow up, preventive and chronic care, care coordination and navigation, behavioral health crisis support, and connections to primary care, specialty care and community resources. Allowable costs include personnel, community paramedicine training and certification, equipment, supplies and technology necessary for service delivery, including vehicles, tablets, Wi Fi routers, documentation and care coordination software, and medical supplies. Indirect costs are capped at 10 percent of total costs, and total administrative costs funded through direct and indirect costs cannot exceed 10 percent of the award.

The Nonagon N9+ device and platform can have a legitimate role in a compliant community paramedicine workflow involving guided remote physical examinations, vitals and telehealth enabled clinical encounters when the applicant, medical director and Kentucky determine that the technology is operationally appropriate and allowable in an approved scope and budget. Dr. Miltie, LLC is not the eligible prime applicant. No new outreach should occur while the state lists the application opportunity as closed.

Application requirements

The 2026 RFA limited the application packet to 25 pages and directed submissions to jimmie.hampton@ky.gov with the required RHT CP RFA subject line. Required elements included agency and applicant information, current community paramedicine status, funding pathway, program evaluation, narrative, sustainability planning, behavioral health partner identification, an attestation, authorized agency representative signature, the required application form and budget template, and supporting readiness documentation such as letters of support. Because Kentucky now lists the opportunity as closed, these instructions should not be used for a new submission unless the state reopens the round or issues updated instructions.
