DDD can change service-specific solicitation status, so vendors must recheck the live DES page before applying.
Deadline and Submission
No fixed calendar deadline, clock time, or time zone is published for this rolling RFQVA. Responses may be submitted while the request is posted and identified as open. Applications are submitted electronically through the DES Contract Administration System (CAS). Procurement inquiries under Part 4 are directed to DDDContractsManager@azdes.gov.
Funding, Rates and Match
This is a vendor procurement, not a grant. Compensation is service-rate based under the DDD Rate Book; Atlas did not verify an aggregate contract ceiling. Grant match or cost share is therefore not applicable.

Service-specific qualifications are controlling. For example, Nursing carries RN/LPN and service requirements, while the Home Health Aid structure requires the applicable Arizona ADHS Home Health Agency licensing and Medicare certification described in the RFQVA. Atlas does not infer that any organization is eligible without verifying the exact selected service and its credential requirements.
Application Review and Amendments
Under the current RFQVA, DDD notifies an applicant within 30 days when an application is minimally inadequate, with a 15-day correction period. Qualified Vendor decisions are made within 60 days after a complete application and baseline process. The RFQVA allows only one Qualified Vendor Agreement per applicant/FEIN.
The current package is identified as RFQVA DDD-2024 Complete Document Effective September 1, 2025. Amendment #1 is effective September 1, 2025. Amendment #2 is the annual renewal, and existing Qualified Vendor Agreements were extended for January 1 through December 31, 2026 under Amendment #2.
