Open CMS Digital Health Participation Pathway
The Centers for Medicare & Medicaid Services (CMS) is accepting applications from patient-facing health apps and technology-enabled care services for inclusion in the Medicare App Library. CMS describes the library as a trusted directory where people with Medicare can discover vetted digital-health options. This is not a grant, cooperative agreement, procurement, or contract award, and CMS does not publish an award value for participation.
Application Status and Deadline
The CMS application page is currently active and provides a “Start application” pathway. CMS does not publish a closing deadline on the controlling application page. Applicants should confirm the live CMS application page before relying on timing.
Who Should Apply
CMS invites apps that integrate with CMS Aligned Networks and support one or more of its initial use cases: replacing manual clipboard workflows with FHIR-based data exchange, conversational AI assistants using clinical records, or diabetes and obesity prevention and management. CMS uses the term “apps” broadly to include traditional mobile or web applications, technology-enabled care services, digital-health platforms, and innovative care-delivery tools.
Core Requirements
Applicants must first sign the CMS Health Tech Ecosystem Pledge. CMS requires secure patient identity verification meeting IAL2/AAL2 standards through CMS-approved services such as ID.me or CLEAR, FHIR R4 and SMART on FHIR interoperability, integration with a CMS Aligned Network, and independent third-party vetting through either the DiMe Seal or the CARIN Alliance/DirectTrust pathway. Patient-facing apps must also meet Medicare connectivity, CMS review, trial-access when applicable, discovery-experience, and HIPAA requirements when operating as a covered entity or business associate.
ACCESS Model Linkage
CMS states that ACCESS Model participants that pledge to join the Health Tech Ecosystem receive a special Medicare App Library designation. ACCESS participants are exempt from CMS Review and Trial Access requirements because those items are addressed through ACCESS participation requirements.
N9+ and Dr. Miltie Fit
N9+ Fit Score: 82/100. The legitimate pathway is as a technology-enabled care service or digital-health platform supporting remote physical examination, vitals, telehealth-enabled encounters, chronic-care support, and structured clinical-data exchange. CMS does not state that N9+ itself is approved, and Atlas has not verified that Dr. Miltie, N9+, or QC Healthcare currently satisfies the required identity, interoperability, accreditation, privacy/security, clinical-evidence, usability, equity, or CMS Aligned Network requirements.
Direct Dr. Miltie participation status: Potentially eligible, subject to capability verification. A capability-gap review should be completed before any application is advanced.
Atlas Analysis
Atlas Opportunity Score: 88/100. This is a strategically strong direct federal participation pathway for digital-health visibility and Medicare beneficiary discovery, but it provides no grant or contract funding. Value depends on Dr. Miltie or QC Healthcare being able to satisfy the technical, privacy, accreditation, and evidence requirements without overstating current readiness.
