Opportunity Overview
CommonSpirit Health’s 2027 Community Health Improvement Grants are currently open for eligible Utah nonprofits serving communities around participating Holy Cross hospitals. The program supports defined 9-12 month community-health projects that address significant needs identified through each hospital’s current Community Health Needs Assessment or implementation strategy and that serve vulnerable or underserved populations.
Deadline and Grant Period
The controlling Mountain Region program page lists the application period as July 20 through September 18, 2026. CommonSpirit does not publish a specific closing time or time zone on that page, so applicants should not assume end-of-day acceptance. Funding decisions are expected on or about January 20, 2027. Funded projects run from April 1, 2027 through March 31, 2028.
Utah Award Ranges and Participating Hospitals
The current 2027 Hospital List for Grant Applicants identifies three Utah application pathways. Holy Cross Hospital – Davis in Layton has a fixed $35,000 grant award, with priorities of access to resources and services and mental, emotional and social well-being. Holy Cross Hospital – Mountain Point in Lehi has a fixed $17,500 grant award, with priorities of access to healthcare and mental health. Holy Cross Hospital – Jordan Valley, Holy Cross Hospital – West Valley and Holy Cross Hospital – Salt Lake grant together as a cluster with awards of $20,000-$35,000 and priorities of mental health and social determinants of health. Ashleigh Phillips is listed as the Local Grant Leader for the Utah opportunities.
Who Can Apply
Applicants must be IRS 501(c)(3) nonprofit organizations in good standing, or use a qualified 501(c)(3) fiscal agent capable of administering the grant. Projects must operate in the relevant hospital service area, address a current CHNA or implementation-strategy priority, serve underserved or vulnerable populations, and provide sustained activity for approximately 9-12 months.
Collaboration and Budget Rules
Requests of $20,000 or more must include substantive collaborating partner organizations other than the granting hospital. The applicant budget must show the overall project budget and other funding sources supporting the project when applicable. No mandatory non-Federal match or cost-share requirement is stated in the reviewed public program page and FAQ.
CommonSpirit limits equipment funding, including computer equipment, and does not fund unrestricted general operating support, event sponsorships, projects shorter than nine months, capital projects such as buildings or renovations, lobbying, political advocacy or political campaigns. The current FAQ also states that the grant program does not fund contracts for services; proposals structured as delivery of services or products to a hospital or its patients at a specified price are contract opportunities rather than eligible grant requests.
Application Requirements
Applications are submitted online through the hospital-specific link in CommonSpirit’s current Hospital List. No emailed applications are accepted. Applicants must provide the required organizational and tax documentation, including an IRS determination letter and a dated, signed March 2024 IRS Form W-9 certifying 501(c)(3) status, plus applicant-controlled project, partner, outcome and budget information. Applicant-specific facts and commitments must be supplied by the applicant and should never be inferred or fabricated.

Atlas Opportunity Score: 92/100. This is a strong Utah community-health funding pathway with multiple hospital markets, clear current award amounts and direct alignment to mental health, access to care and social determinants of health.
N9+ Fit Score: 32/100, conditional. The Nonagon N9+ remote physical examination platform could only fit as a limited, applicant-led technology component where a specific access-to-care project and the live application budget rules support it. N9+ should not be positioned as a stand-alone vendor sale or hospital service contract because service contracts are expressly ineligible and equipment funding is limited. QC Healthcare Fit: 5/100. Dr. Miltie is not an eligible direct lead applicant under the published 501(c)(3) requirement; any legitimate role would be applicant-led and conditional on the grant rules.

Fourth Street Clinic is a realistic high-fit Utah applicant candidate for the Salt Lake cluster because it is a Utah 501(c)(3) healthcare organization serving people experiencing homelessness and provides primary medical, behavioral health, dental, specialty, pharmacy, street-medicine and mobile services. Laurel Ingham, Chief Development Officer, is a publicly verified development and grant-writing leader at the organization. Applicant intent, project scope, budget, partner commitments and submission authority remain applicant-controlled and must be confirmed before any application activity.
