Opportunity Overview
Wisconsin Department of Health Services Chronic Disease Prevention Program is seeking qualifying Wisconsin local and Tribal health departments to establish and expand access to the National Diabetes Prevention Program in rural or geographically hard-to-reach communities. Applications are due September 22, 2026 at 11:59 p.m. CDT.
Funding
Eligible organizations may request up to $15,000. Funding comes through Wisconsin DHS under CDC cooperative agreement CDC-RFA-DP-23-0020. The award uses cost reimbursement. DHS does not state a mandatory match or cost-share requirement in this solicitation. Selected applicants must use GEARS, obtain a STAR Supplier identification number and GEARS Agency number, and have or obtain a UEI before grant agreement execution. A full SAM.gov registration is not required solely to obtain the UEI.
Eligibility
Applicants must be Wisconsin local or Tribal health departments. They must currently have at least one National DPP Lifestyle Coach trained by an organization with a signed CDC MOU, must not already be a CDC-recognized National DPP delivery organization, must complete the National DPP Organizational Capacity Assessment, be prepared to apply for CDC DPRP recognition, and be capable of launching a first cohort by June 1, 2027. DHS also requires capacity to recruit participants, collect and report DPRP data, work with health systems and community organizations, provide appropriate in-person or distance-learning infrastructure, and develop a sustainability plan.
Allowable and Unallowable Costs
Allowable costs include staff time, fringe benefits, project travel, data collection and reporting, project meeting expenses, public health evaluation, office supplies, and consultant or contract services needed to implement the project. DHS identifies clinical care, patient treatment, medications, research, construction, food, unrelated costs, and capital expenditures or capital equipment with an acquisition value above $5,000 and a useful life longer than one year as unallowable.
Submission
The application must be submitted through the Wisconsin DHS Alchemer application by September 22, 2026 at 11:59 p.m. CDT. Applicants must provide the requested narrative responses, proof of Lifestyle Coach training, a completed work plan, and a budget and justification using the DHS templates. DHS states the application must be completed in one sitting.
