The New Jersey Department of Community Affairs SAGE grant portal lists the NJ FamilyCare Hospital and Housing SSI 2026 opportunity, developed with the New Jersey Department of Human Services Division of Medical Assistance and Health Services, to fund partnerships between healthcare settings and NJ FamilyCare Housing Supports Program providers.
Purpose and eligible applicants
The pilot is designed to identify, engage and connect NJ FamilyCare members experiencing homelessness, housing instability or other complex housing needs to ongoing Housing Supports Program services. The application must be a joint New Jersey healthcare/Housing Supports Program partnership; neither party may apply alone. The nonprofit Housing Supports Program provider is the SAGE primary applicant and the participating hospital or healthcare entity is a subrecipient. The healthcare entity must operate in New Jersey and hold a valid NPI. The HSP provider must show substantial operational readiness, including current HSP participation, approved authorizations and claims, staffing capacity, and required Housing First/PWLEE commitments.
The controlling DCA Office of Homelessness Prevention RFP sets the project period at December 1, 2026 through June 30, 2028 and requires a completed DCA SAGE application no later than 11:59 p.m. on October 9, 2026. The RFP does not separately label the timezone, so applicants should use the live SAGE portal as the final submission clock.
Allowable costs and required technical assistance
DCA expressly allows technology, business or operational-practice development, workforce development, and outreach, education and stakeholder convening that directly support the pilot. Allowable technology examples include laptops, tablets, secure mobile devices, referral and data-sharing tools, EHR modifications, virtual-engagement software, related equipment, onboarding consulting and staff training. The controlling RFP sets a standard maximum of $500,000 per pilot site; requests above that amount require written justification and are not guaranteed. The RFP does not state a total program funding pool.
Applicants must attend one mandatory technical-assistance session. The September 10 session has passed; the remaining listed session is September 29, 2026 at 10:00 a.m.. The public SAGE listing does not identify a timezone. The program contact is Rita Mirabelli, 609-913-4761, Rita.Mirabelli@dca.nj.gov.
Application and compliance requirements
Submission is electronic through DCA SAGE. Required threshold materials include current SAM registration/UEI documentation, certifications for debarment/suspension and lobbying, board resolution, IRS determination letter, organizational chart, project budget, executed cover page, a joint-partnership LOI or equivalent, the project narrative, financial/audit materials, bylaws, staffing documentation and resumes. The healthcare partner must provide an NPI confirmation and leadership/institutional commitment documentation. The controlling RFP does not state a mandatory applicant match or cost-share requirement.
Applicant considerations
This is a significant New Jersey Medicaid, hospital and social determinants of health integration pilot with explicit technology allowability. Any proposed technology component must directly support an eligible hospital and Housing Supports Program partnership’s care coordination, referral, documentation, or engagement workflow and must fit the controlling RFP’s allowable cost categories. The program should not be treated as stand alone clinical equipment funding.
