County of Hawaiʻi Program Year 2027 Emergency Solutions Grant (ESG)

Funding: The County ESG webpage summarizes the anticipated Program Year 2027 allocation as approximately $250,000. The current detailed PY2027 proposal packet states approximately $255,000 is anticipated for eligible projects and programs. The County may select one or more proposals and fund an eligible proposal in whole or in part. No per-applicant maximum is published.
Eligible applicants: Private nonprofit organizations authorized to conduct business in Hawaiʻi that propose eligible ESG activities for eligible individuals and families in Hawaiʻi County. Applicants must meet the RFP’s federal and County requirements, including required UEI and SAM.gov registration, Hawaiʻi Compliance Express vendor compliance before agreement execution, and ESG program compliance.
Match: The PY2027 RFP contains a material internal inconsistency that applicants should resolve with the County before finalizing a proposal. Section 5.5 states that each selected subrecipient must provide eligible matching contributions at least equal to the ESG funds awarded to that subrecipient, while the threshold section separately states that the proposal must identify eligible matching contributions equal to the total ESG allocation made to the County. Atlas does not infer that these two standards are equivalent. Applicants should obtain written OHCD clarification and document the source, value, availability, and eligible use of the match required for their proposal.
Submission: Complete proposals are due November 16, 2026 at 4:30 p.m. HST. The current RFP requires a password protected .zip containing the proposal in PDF or Word form to be emailed to ohcdcdbg@hawaiicounty.gov, followed by a telephone call to OHCD by the same deadline to provide the zip password. The password must not be emailed.
Eligible activities: Street outreach, emergency shelter, homelessness prevention, rapid re-housing, and HMIS-related data collection. Street-outreach essential services can include engagement, case management, emergency health and mental health services, transportation, and services for special populations. Emergency-shelter essential services can include outpatient health, mental health, substance-use treatment, case management, child care, education, employment assistance, transportation, and related services.

Dr. Miltie direct applicant: No. The RFP requires an eligible private nonprofit applicant. The RFP’s match provisions contain the internal inconsistency described above, so Atlas will not state a single controlling match amount until OHCD clarifies it. Any Dr. Miltie or QC Healthcare role would need to be a properly procured and allowable vendor or subcontractor role.
Important N9+ caveat: The Nonagon N9+ device and platform could potentially support field based clinical encounters within an eligible Street Outreach health service model. HOPE Services Hawaiʻi already operates Street Medicine with licensed medical personnel and field physical examinations. The current RFP, however, does not specifically authorize device or recurring platform costs. Any proposed cost must be necessary, reasonable, properly allocated to an eligible ESG activity, and accepted by OHCD, so N9+ remains a conditional rather than outreach ready fit.
