Opportunity Overview
The Blue Cross Blue Shield of Michigan Foundation Community Health Matching Grant Program supports Michigan nonprofit community organizations working with health professionals and community leaders to develop population-led, community-inspired programs or implement evidence-based programs addressing community-identified needs with measurable health outcomes.
Funding and Deadline
The program offers up to $25,000 per year for two years; typical awards are $15,000 per year for two years. For the Winter 2027 schedule, the concept paper is due October 28, 2026 at 11:59 p.m. and full applications are accepted until January 6, 2027 at 11:59 p.m.. The controlling public page states that all deadlines end at 11:59 p.m. but does not publish a time zone, so Atlas does not infer one.
Eligibility and Match
Applicants must be nonprofit community organizations based in Michigan. A mandatory match applies and the applicant cannot serve as its own match. For organizations with budgets under $10 million, the required match is 25% when less than 30% of the patient population is Medicaid/uninsured and 10% when more than 30% is Medicaid/uninsured. For organizations with budgets over $10 million, the required match is 60% when less than 30% of the patient population is Medicaid/uninsured and 25% when more than 30% is Medicaid/uninsured.
Application and Cost Rules
The applicant first requests a concept paper through the Foundation’s published concept-paper form. If invited forward, the proposal package includes the project narrative, participant enrollment table, detailed budget and justification, co-funder letters, project timeline, resumes, organizational mission/history/services, IRS 501(c)(3) determination letter, and the most recent tax return or audited financial statement. The Foundation does not support computer equipment including hardware and software, one-time consumables such as medical and office supplies, furnishings, or capital expenses. Indirect costs are not paid except for limited fringe benefits; tuition and continuing education units are excluded, and conference travel is capped at $1,000.

N9+ Fit Score: deferred. The program has a strong healthcare-access focus, but the current budget rules expressly exclude computer hardware/software and capital costs. A narrow programmatic role could be evaluated only if an applicant already has allowable resources and the Foundation confirms the proposed structure. Dr. Miltie, LLC is not an eligible prime applicant. Any QC Healthcare or other contracted-service role would require applicant-specific allowability and budget confirmation.
