Scope

Key Qualifications and Requirements
The controlling RFP requires applicable New Mexico Board of Social Work Examiners licensure as LISW or LCSW, Supervisors Certification, New Mexico Public Education Department social-worker licensure, a completed Medicaid provider-number application or assigned provider number, confidentiality documentation, service and billing documentation, and three recommendations or references from clients receiving similar services.

The official cost response is titled Social Work Services and includes a separate Tele-Therapy per Hour pricing line. Tele-therapy is therefore contemplated as a pricing option within this social-work procurement.
